This combination is indicated as a supplement to support bone mineralization, prevent osteoporosis, and correct deficiencies of calcium, magnesium, and fat-soluble vitamins D3 and K. Vitamin D3 enhances calcium absorption; vitamin K2 is essential for osteocalcin carboxylation and proper deposition of calcium into bone matrix.
Contraindicated in hypercalcemia, vitamin D toxicity, or severe renal impairment. Vitamin K supplementation is relatively contraindicated in patients receiving warfarin anticoagulation therapy, as it may antagonize the anticoagulant effect.
Generally well tolerated at recommended doses. Calcium may cause constipation, bloating, or gas. Excessive vitamin D3 can cause hypercalcemia. Magnesium in high doses may cause diarrhea. Vitamin K is generally safe, though it may interact with anticoagulant therapy.
Regular monitoring of serum calcium and vitamin D levels is recommended during long-term supplementation, especially in the elderly or those with renal disease. Excessive supplementation beyond recommended doses provides no additional benefit and may be harmful.
Calcium and phosphate form hydroxyapatite crystals in bone matrix. Vitamin D3 (cholecalciferol) is hydroxylated in the liver and kidney to the active form 1,25-dihydroxyvitamin D3 (calcitriol), which upregulates intestinal calcium absorption. Vitamin K activates carboxylation-dependent bone proteins. Magnesium is a cofactor in bone metabolism and vitamin D activation.
